1. Application ID / Reference Number

• What to enter: The unique code you received when you submitted your tablet application.
• Format tips:
  • This is usually a mix of letters and numbers (e.g., TBLT-20250428-1234).
  • Check your confirmation email or printed receipt—copy it exactly, including hyphens.

2. Last Name

• What to enter: The last name (surname) you used on the original application.
• Format tips:
  • Enter only letters—no prefixes (e.g., “van,” “de”) unless they were part of your official application.
  • Do not include Jr., Sr., III, etc.

3. ZIP Code

• What to enter: The five-digit postal code for the address on your application.
• Format tips:
  • Use numerals only (e.g., 30318).
  • Do not add the “+4” extension.

4. Date of Birth

• What to enter: Your birth date exactly as on your government ID.
• Format tips:
